Unity.FM Music Agreement

Unity.FM is honored to share your music with our listeners. There are two ways we can feature your music on this internet radio station: live broadcast or live broadcast and archive.

1) Broadcast Live: Your music will broadcast live on Unity.FM during our music broadcasting hours, but will not be archived.
2) Broadcast and Archive Music: Your music will broadcast live on Unity.FM during our music broadcasting hours and may be included in programs that will be archived on Unity.FM. Please note: Archived programs may be downloaded by our listeners for personal use via iTunes and RSS feeds or directly from the Unity.FM Web site.

You may withdraw your permission at any time by sending your request via email to info@unity.fm. Please specify the music to be removed and allow 30 (thirty) days for removal.
Note: No music from any artist will be aired on Unity.FM without permission from the artist and the completion of the Unity.FM Music Agreement.

Please fill out one of the forms below giving Unity.FM permission to either 1) Broadcast your music or 2) Broadcast and Archive your music. To grant your permission now, please complete the form below. If you have questions, please call Unity.FM at 816.347.5553.

1) PERMISSION TO BROADCAST MUSIC
Agreement: I, the above named artist, do grant permission for Unity.FM to broadcast all of my musical works royalty free with the exception of the musical works noted below. I warrant and represent that my musical works are original and not subject to ownership claims of others and that I own the copyright and/or have the right to give this permission for all my musical works. I understand that I retain all other rights to the works and their copyright and that I may withdraw this permission at any time by sending an email to info@unity.fm.

As the artist, you have the option to exclude or include titles. Please complete the section below the agreement accordingly to either allow Unity.FM to broadcast only select titles (box #1) or to broadcast all of your music with the exception of noted titles (box #2).

I understand by choosing "I agree" in the selection below constitutes my electronic signature and therefore binds me to the terms and conditions herein.

This agreement is intended for music used for broadcasting only, not podcasts.
